Build forms and surveys

Create custom forms, embed them anywhere, collect submissions into your CRM, and gather feedback with surveys.

Forms let you collect information from leads and customers. Surveys are a separate tool for gathering feedback.

Build a form

Open Forms and create a new one. Start from a template (like a simple contact form or a quote request) or build your own. In the builder you add fields, name, email, phone, message, and more, and mark which are required.

Map fields to contacts

Map fields to your CRM so submissions create or update the right contact automatically. For example, the email field can map to the contact's email, so you're not re-typing anything.

Publish and share

When the form is ready, click Publish. The button then becomes Share, which opens a screen with the form's shareable link and its embed HTML. Paste the embed code into your website or a landing page.

The Forms list also has a one-click Embed button on each row, so you can grab the code without opening the builder. You can have your CRM email you when a submission comes in.

Collect responses

Every submission is saved and counted on the form. Review them on the form's Responses tab. Form responses are read in the app, there is no export for them yet.

Surveys

Surveys live under Surveys and work differently from forms. They add question types forms do not have, including Star Rating and NPS, and they report an aggregate summary rather than a list of leads. A survey does not map to your CRM, so it does not create contacts. Share one by email or embed it on a page, and export its responses to CSV when you need the raw data.
